Request Process
To request disability services, please complete the following steps:
- Complete the Online Request form
- Submit documentation of your disability. Students may use our Medical Provider Form or provide similar documentation directly from their provider. that includes the following:
- Diagnosis and/or statement of a qualifying disability
- Description of impact/functional limitations and barriers due to the disability
- Signature and credentials of a qualifying health care provider or professional
- Once the steps above are complete, you will be contacted to schedule an intake meeting with the Disability Services Coordinator, Suzy Travis. During this meeting, you will learn more about disability services and discuss any accommodations you might need as a UH Hilo student.

If you would like to receive disability accommodations at UH Hilo, you will need to submit documentation of your disability after you have been admitted to the university.
Documentation should come from a qualified healthcare or other licensed professional and help us understand:
- Your diagnosed and/or qualifying disability.
- How does it impact your learning or participation at UH Hilo?
- Why are the requested accommodations needed?
- Documentation may include medical records, evaluation reports, letters from a provider, or other relevant records. Educational or psychological evaluations are especially helpful when requesting academic accommodations.
Every student and every disability is different. We review each request individually and work with you to determine reasonable accommodations that provide equal access to your education.
Some temporary conditions may not qualify for services through Disability Services. If that's the case, we'll help connect you with other campus resources that may be able to assist.
