Student Medical Services Policy on Excuse Notes

Health Clearance Requirement

If you are planning to enroll in only online courses, you can disregard the regular health clearance requirements/vaccines.

If you are planning to enroll in in-person or hybrid courses, you must submit all your health clearance documents to Student Medical Services before being able to register:

The State of Hawaii Department of Health (DOH) Hawaii Administrative Rules, Title 11 (Chapter 157 and 164.2) mandates health requirements be met in order to attend a post-secondary institution. New requirements are reflected on the Health Requirements Form effective July 1, 2020.

  • Continuously enrolled (Fall to Spring or Spring to Fall) students transferring within the University of Hawaii system, who have had no break in enrollment, will NOT be required to resubmit health clearance information for registration purposes.
  • Students changing home institutions, and Auto Admit recipients within the University of Hawaii system will NOT be required to resubmit health clearance information for registration purposes.
  • Beginning with the Fall 2020 semester, students entering campus housing for the first time (21 years or younger) will need to submit the record of the Meningococcal Conjugate Vaccine. Age exemptions will be given to students 22 years and older.
  • Returning or graduate students who have taken a semester off will need to submit the new immunization requirements.

Send health clearance documents to Student Medical Service via UH Filedrop using the steps below:

  • Save a copy of what you would like to upload on your device or jump drive
  • Log in to the UH File Drop service
  • Specify uhhsms@hawaii.edu as the recipient, click "Proceed"
  • Use "Choose File" option to locate the saved file, click "Start Upload"

About HW Holds

Having an HW hold makes students eligible to register for online classes only. To register for in-person or hybrid classes, you must submit all outstanding health clearances to the UH Hilo Student Medical Services Department. The Health Requirements Form (PDF) is available to ensure all clearances are completed. Please contact the Student Medical Services Department for questions about which clearances are still outstanding.

Telehealth Appointments

All Telehealth appointments require the completion of the Telehealth Informed Consent (PDF) and sent before your appointment using UH File Drop to recipient: uhhsms@hawaii.edu, Fax: 808-932-7368, or mail to UH Hilo Student Medical Services 200 W. Kawili street, Campus Center Room 212, Hilo, HI 96720.

Policy

  • You must be seen within a reasonable time relative to the missed exam, project, class, conference, work, etc.
  • Excuse notes are given at the discretion of the SMS staff based on their medical findings and professional assessments.
  • Excuse notes are only given when care is received through SMS. Please acquire a note from the place of service when care is received elsewhere.
  • The SMS staff does not have the authority to exempt students from class, exams, etc. Such final decisions will be made by the professor, faculty, department, etc.
