Requesting modification of a GE requirement
When you wish to request to
- apply a course not certified for GE to a GE Basic, Area, or Integrative requirement
- waive a GE Basic, Area, or Integrative requirement
- Consult with your faculty advisor or an advisor at the Advising Center
- Complete the form Request for modification of academic requirements
- Fill out the top section
- Then
- For non-certified course approval, fill out the "substitution" section, OR
- For waiver of requirement, fill out the "waiver" section"
- Complete the justification section
- Sign
- Advisor signs the form and keeps a copy
- Take the completed, signed original form to the Dean of your college for review and signature.
- If the Dean approves, arrange to pick up the signed form and deliver it to the Office of the Registrar. Keep a copy.

Requesting modification of a major/minor certificate requirement
When you wish to request to
- substitute another course or courses for (a) major/minor/certificate requirement(s)
- waive (a) major/minor/certificate requirement(s)
- Consult with your major advisor
- Complete the form Request for modification of academic requirements
- Fill out the top section
- Then
- To substitute a course or courses, fill out the "substitution" section, OR
- To waive a requirement, fill out the "waiver" section"
- Complete the justification section
- Sign
- Advisor signs the form and keeps a copy.
- Take the origina signed, completedl form to the program/department chair.
- Chair reviews, approves, signs, checks "major/minor/certificate" and keeps a copy of the form. If multiple programs are involved, the chair of each program/department must review, approve, sign, and keep a copy of the form.
- Take the original completed and signed form to the Office of the Registrar. Keep a copy.
